For a Safe and planned transition from Hospital to the Community.
The service offers occupancy of 13 beds for service users of adult age who may be liable to be deprived of their liberty or may be admitted on a Community Treatment Order (CTO) or Guardianship Order. The Service is divided into 2 apartments with separate facilities such as activity room, clinic, laundry and kitchen.
Apartment 1 is ground floor accommodation and all bedrooms have en-suite bathrooms. The apartment will cater for service users with complex needs and behaviours that challenge, who may be going into crisis. It will act as a gateway for service users who may otherwise be admitted into hospital. Staff on this unit will support the service users to minimise the risk of them going into full crisis.
Apartment 2 is also situated on the ground floor with separate staffing and facilities. This apartment will accommodate service users with complex needs and behaviour who are more stable in mental state but still requires full support for their daily living for a short term or longer term rehabilitation input.
In Maryfield court service users will follow a pathway of care using a recovery star and positive behaviour support model of care to ensure that our service users reach their goals and optimum level of their individual re-ablement. Our maximum accommodation stay is 36 months, within which service users must have reached their full potential. All service users will be regularly reviewed and outcomes measured to ensure progress is monitored and followed through pathways.
