For a Safe and planned transition from Hospital to the Community.
Maryfield Court is situated in the Whalley Range area of Manchester, offering 24 hours nursing care for individuals with a learning disability and or Autism who have complex needs. The model of the service can be used as a step down service enabling a safe transition in to the community from a low secured or rehabilitation hospital. The service can also be used as a step up service in times of crises, acting as a gateway in preventing people with complex needs going into hospital.
The service offers an occupancy of 27 beds for service users of adult age who may be liable to be deprived of their liberty or may be admitted on a Community Treatment Order (CTO) or Guardianship Order. The Service is divided into 4 apartments with apartment 1 and 2 on the ground floor and apartment 3 and 4 on the first floor. Apartment 1 and 2 will accommodate service users with Behaviours that challenge and service users with physical disabilities respectively. Apartment 3 and 4 will accommodate service users who are more stable and require short term or long term rehabilitation input.
Maryfield Court is designed to provide a specialist service within a care home setting, provided by full and competent Multi-disciplinary team (MDT). The MDT professionals will include team of qualified nurses (RMN, RNLD, and RGN), Consultant psychiatrist and SHO, Psychologist, Occupational Therapist and OT Assistants as well as Speech and Language Therapist.
